At the Elder Law Office of Tully Law, P.C. we use a model called The Elder Care Continuum to help families understand the natural progression of aging and its impact on a loved one’s health, mobility, housing and financial resources.

The Elder Care Continuum helps families view the care journey as a process that begins long before family members are forced to scramble to find ways to pay for long-term care.  Our goal is to help families identify where a families loved one might be on the Elder Care Continuum. Integral to this journey is the use of the firm’s Elder Care Coordinator.

An Elder Care Coordinator is a professional, such as a social worker, counselor, nurse, or gerontologist who specializes in assisting older people and their families to attain the highest quality of life given their circumstances.
